Care Management

Support when you need it most

Facing a serious health event or multiple medical issues on your own can be a challenge. The UCare Care Management team can help. Our registered nurses and community health workers offer short-term, one-on-one phone support for many health concerns.

Care Management is available to eligible members at no additional cost.

What we offer

Once you're enrolled, Care Management can provide you or your caregiver with:

  • Disease-specific education
  • Resources for transportation, food, housing, prescriptions and more
  • Help coordinate appointments with doctors and other providers
  • Referrals to additional programs and services

Is Care Management right for you?

You may need Care Management if you have:

  • A newly diagnosed cancer, health condition or rare illness
  • Concerns about managing a complex health condition
  • Multiple hospital stays or a recent hospital discharge for a serious condition

Care Management is open to eligible UCare Medicare Plan, Individual & Family Plan, and Prepaid Medical Assistance Plan (PMAP) members. Contact the Care Management team to learn more or enroll. Medicare and Individual & Family Plan members call 612-676-6538. PMAP and MinnesotaCare members, call 612-676-6512. You can reach us 9 am – 5 pm, Monday through Friday.

Maternal and Child Health Care Management

Pregnancy, postpartum and NICU support

If you’ve been diagnosed with a pregnancy complication, a high-risk pregnancy or need support after your baby is born, help is available. Maternal and Child Health Care Management services are offered to all eligible UCare members.

Our prenatal and postpartum care managers work closely with you to provide education, support and a personalized care plan. For members with babies in a neonatal intensive care unit (NICU), UCare’s NICU care managers provide additional help navigating the health care system and accessing services and resources.

Maternal and Child Health Care Management services are available to Individual & Family Plan, MinnesotaCare (MnCare), UCare Connect and Prepaid Medical Assistance Plan (PMAP) members.
