You’ll have access to North Memorial Health and M Health Fairview as well as many independent providers. Medicare Part B premium giveback
Get $25 back on your Part B monthly premium when you choose the Care Wise with M Health Fairview & North Memorial plan
Dental
Both plans include dental coverage with a $850 allowance on the Care Wise plan and up to $2,000 of routine and restorative coverage on the Care Core plan
Vision coverage
No need to get a separate policy for vision. Your plan includes coverage for your annual routine visit, with diagnostic eye exams available for a copay. Plus, an allowance of $100 to use toward the purchase of prescription eyeglasses or contacts at any provider.
Hearing aids
Get two TruHearing-branded hearing aids per year, and you pay only a set copay of $699 or $999 for each
Over-the-counter allowance
Get a $75 allowance twice a year for over-the-counter items like cough drops, first aid supplies, pain relief and more at participating locations
